This role focuses on providing Permanent Supportive Housing and other housing opportunities to individuals who have experienced homelessness. The Case Manager will conduct interviews and assessments, work with referral sources to intake new participants, and collaborate with housing navigators and landlords to support participants in obtaining and maintaining housing. A key aspect of the role involves providing ongoing case management, including comprehensive goal planning around education, employment, budgeting, housing, health, and wellness. The position also requires developing and maintaining knowledge of community resources, advocating for participants to receive fair and consistent services and public benefits, and assisting clients in increasing their income through various job readiness and training programs. Additionally, the role involves teaching practical life skills such as household organization, safety, home maintenance, and financial management. The Case Manager will support clients moving into stable housing and advocate for necessary services. Accurate record-keeping, including client files and HMIS data, along with report preparation and participation in various meetings, are essential. The position also involves implementing trauma-informed care and harm reduction best practices, adhering to agency values, and performing other assigned duties.
