About The Position

ASD - A Home for Life (ASD) provides affordable housing and supportive services to individuals living with HIV/AIDS and other medically vulnerable populations. The PSH (Permanent Supportive Housing) Case Manager is responsible for ensuring the long-term residential stability of individuals experiencing homelessness through the Dallas County Voucher & Housing Collaboration. Working in partnership with Housing Navigators and the Dallas County Housing Authority (DCHA), the Case Manager guides neighbors through every stage of the housing process—from initial engagement and eligibility confirmation through voucher application, housing search, lease-up, move-in, and ongoing tenancy support. This role promotes ASD’s mission and goals, maintains the confidentiality of all clients, and upholds the dignity and integrity expected of a position of public trust.

Requirements

  • BA/BS required in social work, human services, or related field
  • Experience working with individuals experiencing homelessness, chronic homelessness, or housing instability is required.
  • Familiarity with HMIS and case management databases; proficiency in Microsoft Word, Excel, and Outlook.
  • Access to reliable transportation required; travel to DCHA offices, community sites, and client homes is expected.

Nice To Haves

  • MSW, LCSW, or three years of equivalent experience preferred.
  • Knowledge of or experience with Housing Choice Voucher (HCV) programs, DCHA processes, or Permanent Supportive Housing models is strongly preferred.
  • Experience working with populations affected by mental illness, substance use disorders, chronic health conditions, or HIV/AIDS is highly desired.
  • Familiarity with Padmission Journey or similar housing/case management platforms is a plus.
  • Fluency in a second language may be required.

Responsibilities

  • Coordinate client care with medical, mental health, substance abuse and other providers as necessary.
  • Provides support, assessment, and case management services to individuals.
  • Engage with residents and service providers through personal intervention / interactions, incorporating information as part of care planning / referrals.
  • Provide options / referrals related to substance use and mental health.
  • Refers residents to appropriate community agencies for further assistance and provides discharge planning to ensure post-resident care.
  • Provides outreach and information to the community regarding agency procedures and health and welfare agencies.
  • Participates in intake planning including, but not limited to, intake calls and off-site interviews.
  • Establish and monitor the care plan and growth path on all residential clients.
  • Work with intra agency resources to maintain a clear ongoing path of success for all residents.
  • Provides outreach and information to the community regarding housing graduates and success strategies proven by program participants.
  • Provide linkages to agency, community and government services and monitor referral completion including state insurance continuation programs, medication assistance programs, benefits/health coverage programs such as, SSI, SSDI, Medicaid, Medicare, housing programs, etc.
  • Maintain current documentation and records of case management services as well as statistical data entry required by the program, in the Homeless Management Information System (HMIS) and agency specific databases as necessary, in a timely manner.
  • Assess client stability and provide referrals to increase level of stability and self-sufficiency.
  • Work closely with all departments to ensure client success.
  • Assists in maintaining current information for client records and completes administrative reporting on a timely basis.
  • Performs income assessment and certification for each resident.
  • Must work physically on site
  • Participate actively in any fundraisers provided by ASD.
  • Respond to calls and/or emergencies at any time.
  • Must work physically on site and in the field as required.
