About The Position

The care model prioritizes better, comprehensive primary care rather than service volume, with providers spending more time with patients and addressing their health needs.

The Per Diem Provider delivers outpatient primary care to a primarily Medicare-aged, medically underserved patient population, including patients with comorbid conditions. Providers work collaboratively with a supportive center team that may include Family Practice and/or Internal Medicine Physicians and RN Clinical Managers. The role may involve travel among nearby centers within the market.

Requirements

• Active, non-probationary license to practice professional nursing or as a Physician Assistant in the state of employment.
• Experience in primary care, such as internal medicine, geriatrics, or family medicine.
• Current BLS/ACLS certification.
• Strong understanding of examination methodologies and diagnostics.
• Interpersonal skills to communicate accessibly with patients, families, and center team members.
• Confidence in delivering high-quality primary care.

Responsibilities

• Provide outpatient primary care to patients.
• Manage a primarily Medicare-aged, medically underserved patient population, including patients with comorbid conditions.
• Address patients’ medical and social needs and be available to patients.
• Work collaboratively with the center team and other clinicians.
• Travel within the market to multiple nearby centers as needed.
