About The Position
Circadia Health is a growth-stage healthcare AI company focused on preventing hospitalizations and transforming senior care. The Circadia Intelligence Platform integrates contactless sensing, predictive models, and enterprise integrations to detect adverse events early and operationalize interventions. The company's technology currently supports over 40,000 post-acute patients daily across various care settings. This role involves working with both skilled nursing and Home Health AI Care programs, utilizing AI-driven insights to provide direct patient care, coach facility teams, and reduce preventable readmissions. The Nurse Practitioner will round on patients in SNF partner facilities and provide care in patients' homes, blending direct patient care with technology integration and training.
Requirements
- Certified and licensed Nurse Practitioner in ME.
- ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
- Eligible for Medicare participation.
- 2+ years NP experience with complex adult populations.
- Able to practice independently.
- Valid ME driver's license.
- Comfortable traveling locally.
- Thrives in a dynamic, evolving startup environment.
- English required.
Nice To Haves
- Tech-savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
- Spanish is preferred.
Responsibilities
- Conduct initial and follow-up assessments on flagged residents.
- Initiate/adjust plans of care.
- Complete consent forms.
- Coordinate with facility providers.
- Train and coach staff at the point of care, standardizing the use of Circadia into clinical workflows.
- Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
- Complete documentation/charts within the same day.
- Efficiently complete visits across designated SNFs (typically 25+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.
- Surface field insights to improve workflows and tech integration in a clinical setting.
- Support NP onboarding, share best practices, and partner with clinical ops/product teams.
- Cover SNF sites and patient homes within a 30-40 mile radius, with mileage and lodging covered for locations outside this radius.
