About The Position
Circadia Health is a growth-stage healthcare AI company focused on preventing avoidable hospitalizations and improving senior-care operations. Its platform combines contactless sensing, predictive models, and enterprise integrations. The technology serves post-acute patients across skilled-nursing, home-health, and home-care networks. The on-site Nurse Practitioner provides proactive care at partner skilled-nursing facilities and in home-health settings. The role combines direct patient care with integrating Circadia technology into clinical workflows through staff training, live demonstrations, and shadowed assessments. The NP rounds on new admissions, facility-flagged patients, and patients identified by Circadia alerts, using predictive health signals to support earlier interventions and reduce preventable readmissions. The position is based in Sun City, Arizona, and involves local travel among designated sites. The per-diem schedule requires a minimum of 4–8 days per month, with opportunities for additional days based on patient demand. Compensation is paid per visit, with a listed monthly range of $3,000–$5,200.
Requirements
- Certified and licensed Nurse Practitioner in Arizona, with ANCC or equivalent certification in Adult, Family, Geriatric, or Acute care.
- Eligible for Medicare participation; the company bills on the Nurse Practitioner’s behalf.
- At least 2 years of Arizona experience with complex adult populations.
- Ability to practice independently.
- Valid driver’s license and comfort with local travel.
- Ability to thrive in a dynamic, evolving startup environment.
- English proficiency required.
Nice To Haves
- Familiarity with Google Suite, multiple electronic medical records, Slack, and AI transcription or other AI tools.
- Spanish language skills.
Responsibilities
- Conduct initial and follow-up assessments of flagged residents; initiate or adjust plans of care, complete consent, and coordinate with facility providers.
- Train and coach facility staff at the point of care and standardize Circadia use in clinical workflows.
- Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
- Complete documentation and charts on the same day; complete notes using AI-powered tools within 24 hours of each encounter.
- Efficiently round across designated skilled-nursing facilities, typically completing 25 or more visits per day where appropriate, with focus on new admissions, facility-flagged patients, and Circadia alerts.
- Provide care in patients’ homes in home-health settings using the same proactive care model.
- Share field insights to improve workflows and clinical technology integration.
- Support Nurse Practitioner onboarding, share best practices, and collaborate with clinical operations and product teams.
- Cover sites within a 30–40-mile radius; mileage and lodging for sites beyond that radius are covered per policy.
- Meet targets for patient visits, documentation completion, facility workflow adherence, and improvement trends in falls and readmissions.
