About The Position

Circadia Health is a growth-stage healthcare AI company focused on preventing avoidable hospitalizations and improving senior-care operations. Its Circadia Intelligence Platform combines contactless monitoring of respiration and motion, predictive models that identify many preventable adverse events in advance, and integrations with clinical and operational workflows. The technology supports more than 40,000 post-acute patients daily across skilled-nursing, home-health, and home-care networks. The Nurse Practitioner works across skilled nursing facilities and the Home Health AI Care program, providing proactive patient care informed by predictive health signals. The NP also supports technology integration through staff training, demonstrations, and shadowed assessments.
