This role involves transforming patient care through AI in both skilled nursing facilities and home health settings. As a Circadia Nurse Practitioner, you will directly engage with patients, act on real-time predictive health signals, and guide facility teams in utilizing data for early interventions to reduce preventable readmissions. Responsibilities include rounding on new admissions and flagged patients in SNF partners, providing proactive care in patients' homes, and integrating technology with direct patient care through training and demonstrations. The position requires flexibility to cover multiple sites across southern and central Maine (Kennebunk, Augusta, Norway, Bath, Brunswick, and Yarmouth) and Dover, NH. Candidates must hold active licensure in Maine and New Hampshire or be willing to obtain NH licensure upon hire.
Requirements
- Certified and licensed Nurse Practitioner in ME & NH.
- ANCC (or equivalent) certification in Adult/Family/Geriatric/Acute.
- Eligible for Medicare participation; company bills on your behalf.
- 2+ years of NP experience with complex adult populations.
- Ability to practice independently.
- Valid ME & NH driver's license.
- Comfortable traveling locally.
- Ability to thrive in a dynamic, evolving startup environment.
- English proficiency required.
Nice To Haves
- Tech-savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
- Spanish language proficiency is preferred.
Responsibilities
- Conduct initial and follow-up assessments on flagged residents.
- Initiate and adjust plans of care.
- Complete consent forms.
- Coordinate with facility providers.
- Train and coach staff at the point of care, standardizing the use of Circadia into clinical workflows.
- Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
- Complete documentation and charts within the same day.
- Efficiently complete visits across designated SNFs (typically 25+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.
- Surface field insights to improve workflows and tech integration in a clinical setting.
- Support NP onboarding, share best practices, and partner with clinical operations/product teams.
- Cover SNF sites and patient homes within a 30-40 mile radius; mileage and lodging are covered for locations outside this radius per company policy.
