Pediatric Transitional Nurse – Registered Nurse (RN) Licensed Practical Nurse (LPN)
About The Position
Angels of Care Pediatric Home Health is hiring Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) for its Pediatric Transitional Nurse Team. The role supports medically fragile children and their families during critical transitions to home care, helping ensure safe discharges, prevent readmissions, provide interim patient coverage, and maintain continuity of care until permanent staffing is established. The nurse provides skilled one-on-one pediatric care in patients’ homes, supports high-acuity patients, and assists families and caregivers. The position is full-time with guaranteed hours, day and night shifts available, on-call and staffing coverage responsibilities as needed, and local travel in a field-based role.
Requirements
- Active, unrestricted Pennsylvania Registered Nurse (RN) or Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) license.
- Current CPR certification.
- Ability to lift and assist pediatric patients as needed.
- Pediatric home health experience.
- Experience with tracheostomy care and ventilator management.
- Experience with high-acuity pediatric patients.

Responsibilities
- Provide skilled one-on-one pediatric nursing care in patients’ homes.
- Perform patient assessments and ongoing evaluations.
- Administer medications, treatments, and therapies.
- Support hospital-to-home transitions for high-acuity patients.
- Educate and support parents, families, and caregivers.
- Assist with discharge support and care coordination.
- Train and support permanent field nurses during transitions.
- Provide first-shift and interim coverage as needed.
- Collaborate with Clinical Care Managers and interdisciplinary teams.
- Respond appropriately to emergencies and changes in patient condition.
