Pediatric Transitional Nurse – Registered Nurse (RN)
About The Position
Angels of Care Pediatric Home Health is hiring a Pediatric Transitional Nurse to provide one-on-one skilled nursing care to medically fragile children during transitions from hospital to home. The role supports safe discharges, helps prevent readmissions, provides interim patient coverage, and maintains continuity of care until permanent staffing is established. Nurses work with high-acuity pediatric patients and their families in patients’ homes, coordinating with Clinical Care Managers and interdisciplinary teams. The position is full-time with guaranteed hours, day and night shifts available, and local field travel of up to one hour. On-call and staffing coverage may be needed. Requirements
- Active, unrestricted Florida Registered Nurse (RN) license; the posting also references a Licensed Practical Nurse (LPN) license in its heading and lists RN or license (FL) in the qualifications.
- Current CPR certification.
- Ability to lift and assist pediatric patients as needed.
- Pediatric home health experience.
- Experience with tracheostomy care, ventilator management, and TPN.
- Experience caring for high-acuity pediatric patients.

Responsibilities
- Provide skilled one-on-one pediatric nursing care in patients’ homes.
- Perform patient assessments and ongoing evaluations.
- Administer medications, treatments, and therapies.
- Support hospital-to-home transitions for high-acuity patients.
- Educate and support parents, families, and caregivers.
- Assist with discharge support and care coordination.
- Train and support permanent field nurses during transitions.
- Provide first-shift and interim coverage as needed.
- Collaborate with Clinical Care Managers and interdisciplinary teams.
- Respond appropriately to emergencies and changes in a patient’s condition.
- Provide on-call and staffing coverage as needed.
