Pediatric Transitional Nurse – Registered Nurse (RN)
About The Position
Angels of Care Pediatric Home Health is hiring a Registered Nurse for its Pediatric Transitional Nurse Team in Port St. Lucie, Florida. The role provides skilled, one-on-one nursing care to medically fragile pediatric patients in their homes, especially during hospital-to-home transitions. The nurse supports safe discharges, helps prevent readmissions, provides interim patient coverage, and maintains continuity of care until permanent staffing is established. The position involves high-acuity pediatric care, including trach, ventilator, G-tube, and emergency management. It is full-time with guaranteed hours, day and night shifts available, on-call and staffing coverage as needed, and local field travel of up to one hour. The listed RN pay range is $37–$40; the posting also lists an LPN range of $34–$37.
Requirements
- Active, unrestricted Registered Nurse (RN) license in Florida.
- Current CPR certification.
- Ability to lift and assist pediatric patients as needed.
- Pediatric home health experience.
- Experience with trach care, ventilator management, and TPN.
- Experience caring for high-acuity pediatric patients.

Responsibilities
- Provide skilled, one-on-one pediatric nursing care in patients’ homes.
- Perform patient assessments and ongoing evaluations.
- Administer medications, treatments, and therapies.
- Support hospital-to-home transitions for high-acuity patients.
- Educate and support parents, families, and caregivers.
- Assist with discharge support and care coordination.
- Train and support permanent field nurses during transitions.
- Provide first-shift and interim coverage as needed.
- Collaborate with Clinical Care Managers and interdisciplinary teams.
- Respond appropriately to emergencies and changes in a patient’s condition.
- Provide on-call and staffing coverage as needed.
