Pediatric Transitional Nurse – Registered Nurse (RN)
About The Position
Angels of Care Pediatric Home Health is hiring a Registered Nurse for its Pediatric Transitional Nurse Team, supporting medically fragile children and their families during transitions to home care. The nurse provides high-acuity, one-on-one pediatric care, helps ensure safe discharges and continuity of care, supports readmission prevention, and provides interim coverage until permanent staffing is established. The position is full-time with guaranteed hours; day and night shifts are available. It is field-based, with local travel of up to one hour, and includes on-call and staffing coverage responsibilities as needed.
Requirements
- Active, unrestricted Registered Nurse (RN) license in Florida.
- Current CPR certification.
- Ability to lift and assist pediatric patients as needed.
- Pediatric home health experience.
- Experience with tracheostomy care, ventilator management, and TPN.
- Experience caring for high-acuity pediatric patients.
- Must submit a resume to be considered.
Responsibilities
- Provide skilled one-on-one pediatric nursing care in patients’ homes.
- Perform patient assessments and ongoing evaluations.
- Administer medications, treatments, and therapies.
- Support hospital-to-home transitions for high-acuity patients.
- Educate and support parents, families, and caregivers.
- Assist with discharge support and care coordination.
- Train and support permanent field nurses during transitions.
- Provide first-shift and interim coverage as needed.
- Collaborate with Clinical Care Managers and interdisciplinary teams.
- Respond appropriately to emergencies and changes in patient condition.
