Pediatric Transitional Nurse – Registered Nurse (RN)
About The Position
Angels of Care Pediatric Home Health is hiring Registered Nurses for its Pediatric Transitional Nurse Team to care for medically fragile children and support their families during transitions to home care. The role focuses on safe hospital discharges, preventing readmissions, interim patient coverage, and continuity of care until permanent staffing is established. Nurses provide one-on-one, high-acuity pediatric care in patients’ homes and may train and mentor other nurses. This is a full-time, field-based position in Jacksonville, Florida, with guaranteed hours. Day and night shifts are available. The role includes on-call and staffing coverage responsibilities as needed, as well as local travel of up to one hour. Trach care, ventilator management, G-tube care, emergency management, and TPN experience are relevant to the role. Applicants must submit a resume to be considered.
Requirements
- Active, unrestricted Florida Registered Nurse (RN) license.
- Current CPR certification.
- Ability to lift and assist pediatric patients as needed.
- Pediatric home health experience.
- Experience with trach care, ventilator management, and TPN.
- Experience with high-acuity pediatric patients.
Responsibilities
- Provide skilled one-on-one pediatric nursing care in patients’ homes.
- Perform patient assessments and ongoing evaluations.
- Administer medications, treatments, and therapies.
- Support hospital-to-home transitions for high-acuity patients.
- Educate and support parents, families, and caregivers.
- Assist with discharge support and care coordination.
- Train and support permanent field nurses during transitions.
- Provide first-shift and interim coverage as needed.
- Collaborate with Clinical Care Managers and interdisciplinary teams.
- Respond appropriately to emergencies and changes in a patient’s condition.
