Improves the overall health of the communities we serve by delivering exceptional patient scheduling and supporting real‑time in‑clinic care team coordination. This role involves providing excellent customer service as a first point of contact for patients, managing calls, and delivering accurate information about services, locations, providers, and access options. The specialist will schedule follow-up appointments, conduct proactive outreach for missed appointments, and manage a patient waitlist to improve access. They will also align visits with provider preferences and guidelines, monitor provider schedules for optimization, and collaborate with care team members. Developing expertise in clinic workflows and scheduling standards is key, as is clear communication of appointment details and documentation in the electronic health record. The role also supports quality improvement initiatives and updates scheduling practices as needed, while upholding the CHAS Health Mission and Core Values.
