About The Position

The patient navigator is responsible for collecting, managing, and reporting required data related to CCBHC required outcomes and measures which include National Outcome Measures (NOM’s) and a required reporting measures for CCBHC. This requires both interpersonal and data management skills. The patient navigator assures that patients are connected with the right program and services and seek to close referral gaps that patients may encounter while receiving care in the CCBHC clinic. The patient navigator must be able to work collaboratively with clients, families, internal programs, external providers, and all aspects of the CCBHC network. The patient navigator must be an outcome focused individual that seeks to help clients have an effective and efficient experience while receiving care at the CCBHC. The patient navigator is responsible for monitoring external sources of data to determine if a patient has had any readmissions to a hospital for psychiatric or substance use reasons in the previous 30 days. The patient navigator is also responsible for monitoring and any necessary follow-up of patients placed on the Zero Suicide pathway.

Requirements

  • Possess demonstrated interpersonal skills.
  • Ability to work with a wide variety of individuals with behavioral health needs served by a CCBHC.
  • Ability to react effectively in a wide variety of human service situations.
  • Good listening skills, verbal skills, and written communication skills.
  • Able to observe patient demeanor and non-verbal communication for assessment purposes.
  • At least 18 years of age.
  • Able and willing to work flexible hours and adjust work schedule to meet patient and organization needs.
  • Must hold and maintain State of Kansas Case Manager Certification and appropriate CEU’s where applicable.
  • Able to communicate effectively with referral sources, external care providers, and mental health center team members (i.e. case managers, therapists, physicians, community support workers, teachers, pastors, coaches).
  • Completion of required trainings upon hire.
  • Work independently, or as a productive member of a team to develop a therapeutic rapport with diverse individuals.
  • Valid Kansas driver’s license.
  • Proficient at basic keyboarding/word-processing skills.
  • Able to pass Kansas Bureau of Investigation Screen, Central Registry Screen, and DCF (Kansas Dept. for Children & Families, formerly SRS) background check.
