About The Position

The patient navigator guides patients throughout the cancer care continuum, from cancer prevention, to treatment, to survivorship, to end-of-life care. This role involves coordinating appointments, facilitating linkages to follow-up services, arranging transportation assistance, and coordinating with financial navigation services. The navigator also maintains communication with patients, survivors, families, and healthcare providers, monitors patient satisfaction, and collaborates with physicians and the multidisciplinary team. Additionally, the role provides non-clinical and emotional support, advocates for the patient, and maintains professional knowledge and skills.

Requirements

  • Bachelors’s degree in health care related field preferred
  • 2+ years of experience working in a medical setting with oncology patients and families.
  • Global perspective of changing health care environment

  • Knowledge about the breadth and accessibility of community resources.

  • May be exposed to hazardous materials and life-threatening diseases
  • Ability to perform lifting/transfer activities related to patients (up to 50 lbs. may be required)
  • Must be able to travel to visit physicians and other practices within our region
  • This position needs to recognize needs and behaviors of specific age groups of patients treated
  • Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.

Responsibilities

  • Coordinates appointments with providers to ensure timely delivery of diagnostic and treatment services.
  • Ensures that appropriate medical records are available at scheduled appointments.
  • Arranges transportation assistance with available resources.
  • Coordinates appointment with financial navigation services.
  • Maintains communication with patients, survivors, families, and health care providers at various touchpoints during their cancer care.
  • Monitor patient satisfaction with the cancer experience and works with team(s) to address any identified issues or barriers.
  • Communicates and collaborates with the treating physician(s).
  • Communicates and collaborates with the multidisciplinary team(s).
  • Receives and responds to new patient referrals.
  • Provides non-clinical and emotional support for patients.
  • Advocates on behalf of the patient and Empowers the patient to self-advocate.
  • Maintains own knowledge, ability, skills, competencies.
