About The Position

The Oncology Patient Navigator works in collaboration and continuous partnership with chronically ill or high risk patients to coordinate care between diagnosis, treatment, survivorship or end of life. He/she participates in the assessment and ongoing support of SKCC patients in close collaboration with the Care Team(s) s/he is supporting.

Requirements

  • High school or GED required.
  • Two + years experience in clinical or community resource settings; care coordination experience is desirable.
  • Must have experience in a healthcare clinical or community setting, experience in an outpatient oncology setting is desirable.
  • Must demonstrate a service-oriented approach and be a team player willing to support any SKCC patient.
  • Required completion of the George Washington University Cancer Center Oncology Patient Navigator Training, a 20 hour self-paced course for non-clinical navigators, and exhibit competency in each of the 8 domains.

  • Bachelor Degree preferred.

Responsibilities

  • Serves as the point of contact, advocate, and informational resource for patients, care team, family/caregiver(s).
  • Promotes timely access to appropriate care such as helping to schedule appointments
  • Gathers records, organizes and supports Tumor Boards and Multidisciplinary Clinic(s) and preparation for effective oncology appointments
  • Monitors and promotes adherence to a care plan, developed in coordination with the patient, PCP, oncologist and family/caregiver(s)
  • Supports communication between the care team and the patient, facilitates screening and addresses barriers to care and documents all interactions in the patient’s electronic record
  • Connects patients to relevant community resources, with goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care costs
  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson
  • Identification and support of high risk patients
  • Facilitate and attend meetings between patient, family/caregiver(s), care team, and shares community resources as needed
  • Responsible for regular outreach to patients being served by the Oncology Navigation Team to check-in, ensure their needs are being met and raise awareness of new or established SKCC offerings
  • Responsible for coordinating services related to patient education, treatment (including tests and labs), support and follow up care
  • Support program growth and patient experience in all patient interactions
