About The Position

The Patient Navigation & Care Coordinator (PNCC) helps patients across Hawaiʻi access timely, compassionate, and coordinated healthcare. Working with the Clinical Nurse Coordinator, case management, and health center teams, the PNCC coordinates travel logistics, facilitates care transitions, and develops community partnerships. The position supports all PPGNHAIK service lines, with particular attention to time-sensitive abortion care coordination, and helps patients transition to ongoing prenatal care through the Bridges to Prenatal Care program. The role provides non-clinical patient navigation, scheduling support, referrals, and connections to community resources. It coordinates inter-island and same-island travel, including flights, transportation, and lodging when needed, and helps patients address practical barriers to care. The PNCC also maintains referral relationships and resource information, documents activities in electronic medical records and program tracking systems, and supports program reporting, administrative processes, quality improvement, research, and evaluation. The position is based on Oahu and requires regular travel to other islands. The schedule is determined around health center needs. This is a full-time, 37.5-hour-per-week, non-exempt position. The listed Hawaii pay range is $23.28–$34.92 per hour; starting pay depends on factors including relevant experience, education, internal equity, geographic location, and the budgeted hiring range.
