About The Position

Compton Care is seeking a proactive and organised Patient Flow Facilitator to support safe, timely and person-centred transfers of care across its Community and Virtual Ward services. The role works closely with patients, families, acute hospital wards, community teams, social care and other system partners to help people receive appropriate care in the right place and avoid unnecessary delays. The postholder will be visible and responsive across acute ward areas, building relationships with NHS colleagues and wider partners to identify patients who may benefit from Compton Care support, remove discharge barriers and maintain effective flow. They will work with multidisciplinary teams, attend board rounds or discharge discussions where required, gather accurate information, escalate delays appropriately and support clear communication with patients and those important to them. The role contributes to safe and effective transitions of care, fewer avoidable delays, improved patient experience and stronger partnership working between Compton Care and NHS and social care colleagues. Regular presence in acute hospital wards and community settings is required, so access to transport and the ability to travel across the local health and care system are essential.
