About The Position

The Patient Continuity of Care Coordinator provides oversight for the communication following the MQSA ACR criteria with patients during the journey of breast care and recommendations for followup indicated by final report. Communication between radiologist, Director, Chief Technologists, Office Manager, and nursing is vital to adhere to regulatory requirements.

Requirements

  • High School diploma or GED required
  • Bachelor Degree preferred

Responsibilities

  • Calls all patients who have been called back from screening mammograms to evaluate a suspicious finding
  • Manages call back worklist and makes notes updating status of each patient called.
  • Manages the category O worklist and Birads 4,5,6 worklist for appropriate communication with patients and leadership.
  • Requests all surgical pathology reports for patients that we have referred to surgeons.
  • Enters/inputs/uploads all pathology reports for patients from both needle biopsies and outside surgeries into the EMR
  • Verifies appropriate follow up is entered for all applicable patients. (ie: screening or diagnostic follow up)
  • Requests clinic notes on all patients who have been referred to other medical professionals (surgeons, dermatologists, oncologists, etc).
  • Sends all referrals recommended by radiologists and follows up with the office to ensure receipt of referral and timely scheduling of patient.
  • Imports all clinic notes received from outside clinics into patient EMR.
  • Determines patients who need certified letters.
  • Label and send certified letters
  • Monitors Pathology Worklist for appropriate communication internally
  • Performs any other related duties as required or assigned.
