Requirements

Responsibilities

  • Presents a friendly, helpful face to patients, people seeking testing or pharmacy patrons.
  • Greets patients in a courteous and sensitive manner.
  • Schedules appointments.
  • Assists with the needs of our clients.
  • Offers patient intake interviews.
  • Conducts financial screenings.
  • Registers for Ryan White and AIDs Drug Assistance Program (ADAP) services.
  • Ensures that appropriate documentation for patient eligibility is filed and entered into our computer database.
  • Accurately collects new patient medical history.
  • Determines what tests/screenings are due during medical visits.
  • Takes patient vitals and documents all patient health-related complaints.
  • Communicates with patients between provider visits for condition updates.
  • Follows through on tests/procedures ordered by providers.
  • Manages the referral process by processing specialty referrals in the most timely and accurate manner to optimize care and to address clinical needs.
  • Provides all pertinent information to patients for timely appointment scheduling.
  • Assists with appointment scheduling as patients require.
  • Obtains specialist consults and scans into the patient’s EMR to provide the most complete picture of a patient’s clinical condition.
  • Tracks and monitors the status and progress of the referral being completed.
  • Notifies a provider of a patient’s lapse in completing the referral.
  • Remains in contact with the patient to improve adherence.
  • Adds consistent value to the Healthcare Center Team especially in high-stress environments that demand immediate attention, by being flexible and professional at all times.
