About The Position
UW Medical Center Montlake - Heart Institute has an outstanding opportunity for a Patient Care Coordinator. This is a full-time, 100% FTE (40 hours per week) position on the Day Shift, working Mondays through Fridays. This is an on-site role with no telework. The Heart Institute at UW Medical Center – Montlake provides a full range of heart healthcare and prevention services, including general cardiology, cardiac diagnostic imaging, cardiac surgery, arrhythmia management, interventional cardiology, structural heart services, advanced heart failure management, heart transplant, VADs, adult congenital heart disease management, cardiac psychiatry, and cardiovascular genetics.
Requirements
- High school graduation or equivalent
- Two years of experience in the program specialty
- Equivalent education/experience may substitute for minimum qualifications except when there are legal requirements, such as a license, certification, and/or registration
Responsibilities
- Triage medical information in a specific medical or surgical specialty
- Schedule post-surgery diagnostic procedures and follow-up patient appointments
- Act as a liaison between patients, consulting physician, referring physician, or outside agency to coordinate patient appointments, admission, or surgery. Will coordinate the exchange of information either written or verbally before and after patients are seen
- Coordinate anesthesia
- Obtain all necessary medical records, diagnostic studies, pathology items, etc., that are needed for patient care
- Assist in scheduling new patient appointments by triaging to appropriate department and physician
- Enter appointment information in online appointment system
- Coordinate pre-admission tests and procedures
- Schedule and ensure pre-anesthesia works-ups are completed
- Coordinate the direct transfer of patients from outside hospitals to UWMC with physicians, medical staff, and Admitting
