Patient Care Coordinator I
About The Position
CVS Health is hiring a remote Patient Care Coordinator I for its Coram Enteral Nutrition Center team. Candidates may live anywhere in the United States. The role supports the Enteral Nutrition department by processing monthly patient refill requests and ensuring documentation is accurate before orders ship. It also supports new patient orders, electronic medical record updates, and the admissions process to help patients begin services with the company. The coordinator answers questions from patients and caregivers about nutrition therapy and admission status, initiates the referral process, and responds to calls from patients, caregivers, branches, and referral sources. The position is full-time, benefited, and not through an agency. Typical hours are Monday through Friday, 8:00 a.m. to 4:30 p.m. CST, with 40 anticipated weekly hours. The listed starting rate is $19.00; the typical pay range is $17.00–$25.65 per hour. The role may also be eligible for a CVS Health bonus, commission, or short-term incentive program.
Requirements
- At least one year of experience working in a customer service environment.
- Experience with data entry.
- Experience using Microsoft Office, specifically Excel, Outlook, and Word.
- High school diploma or GED.
Nice To Haves
- Home infusion or durable medical equipment (DME) experience.
- Call center experience.
- Experience working in a healthcare environment.
Responsibilities
- Process monthly patient refill requests and ensure documentation is accurate before orders ship.
- Process new patient orders and update electronic medical records.
- Work with the patient admissions department to help bring patients onto services with the company.
- Answer incoming calls from patients, caregivers, branches, and referral sources about nutrition therapy order status.
- Triage calls to the appropriate departments.
- Verify patient demographic and insurance information to ensure accurate shipment.
- Initiate the referral process and answer patient and caregiver questions about admission status.
