About The Position

The Patient Care Advocate II (LPN/LVN) is a licensed, field-based member of WellBe’s interdisciplinary geriatric care team. The role delivers high-quality in-home care to frail, poly-chronic, elderly Medicare Advantage patients and combines patient advocacy, care coordination, quality (HEDIS) responsibilities, clinical support, and telehealth visit facilitation. Practicing within the LPN/LVN scope and at the direction of the assigned Provider, the advocate conducts in-person visits, facilitates telehealth consultations with an APP or Physician, captures and documents clinical data, supports patient education, and connects patients with support services across the WellBe platform. The role works closely with provider and social-work teams, has no direct reports, and follows applicable state and federal regulations and certifying-body requirements. Daily local travel to meet patients and engage the community is essential. The Patient Care Advocate II facilitates initial, follow-up, and urgent in-home telehealth visits; records vital signs and observed or reported health changes; conducts home-safety inspections; provides care support and education at the direction of the assigned Provider; performs minimally invasive screenings and point-of-care testing; and, where permitted by law and company protocols, performs blood collection and intravenous therapy. The role also supports patient engagement, appointment scheduling, care transitions, palliative-care needs, medication adherence, HEDIS/STAR quality performance, and accurate daily EMR documentation. The advocate collaborates with the interdisciplinary team, participates in team and IDT meetings, builds relationships with local providers and caregivers, and maintains required licensure and certifications. This is a field-based position involving daily local travel, potentially up to 100%, to patient homes and other care settings such as hospitals, skilled nursing facilities, and assisted-living facilities. It is safety-sensitive and requires employees to perform duties in a way that ensures the safety of themselves, coworkers, patients, and the public. Drug and alcohol testing applies in accordance with applicable laws and company policy; marijuana use is prohibited as described in the posting. WellBe does not provide employment-based visa sponsorship.
