About The Position

The Patient Care Advocate II (LPN/LVN) is a licensed, field-based member of WellBe’s interdisciplinary geriatric care team. The role provides in-home care to frail, poly-chronic elderly Medicare Advantage patients and combines patient advocacy, care coordination, quality (HEDIS) responsibilities, and clinical and telehealth visit facilitation. Working within the LPN/LVN scope of practice and at the direction of the assigned Provider, the advocate conducts in-person visits, facilitates telehealth consultations with an APP or Physician, captures and documents clinical data, supports patient education, and connects patients with local support services. Daily local travel to patients and community locations is essential. The advocate supports initial, follow-up, urgent, and drop-in visits; records vitals; performs home-safety inspections, screenings, and point-of-care testing; and, subject to state law, training, and company protocols, may perform venipuncture, capillary blood collection, and intravenous therapy. The role also coordinates appointments and diagnostic tests, supports patient engagement and transitions of care, assists with HEDIS and STARS quality efforts, and maintains timely, complete records in the EMR. The position is field-based and involves travel to patient homes and facilities, including hospitals, skilled nursing facilities, and assisted living settings. Compensation is $30.00–$38.00 per hour. This is a safety-sensitive position; employees are subject to drug and alcohol testing under applicable law and company policy.
