About The Position

Kindred Hospital Dayton and Behavioral Health Unit (BHU) is a 97-bed hospital. With 51 long-term acute care hospital (LTACH) beds, we offer the same in-depth care you would receive in a traditional hospital, but for an extended recovery period. Our 46-bed behavioral health unit offers treatment for adults ages 18+, including dedicated services for geriatric patients. The unit offers 24-hour supervised psychiatric treatment and crisis support for mental health issues and like the LTACH, we partner with your physician and offer 24-hour clinical care seven days a week so you can start your journey to wellness. We are located downtown Dayton near the I-75 and I-35 junction, to the west of the Great Miami River. This role conducts patient assessments in accordance with company standards for appropriate patients for potential admission into the system. The coordinator maintains positive relationships with referral sources, sales teams, admission, managed care, clinical and operational teams. They coordinate the transmission of clinical and benefits information of patients for possible admission into our ScionHealth Specialty Hospitals. The role involves collecting, organizing, evaluating, and communicating pertinent clinical information with effective verbal and written skills. Strong customer service skills and commitment to service excellence are essential. The position requires strong attention to detail and commitment to following all processes set forth, operating with a sense of urgency to produce quick results. All patient information is maintained in a manner that ensures confidentiality and meets all regulatory requirements.

Requirements

  • Ability to collect, organize, evaluate and communicate pertinent clinical information with effective verbal and written skills.
  • Strong customer service skills and commitment to service excellence.
  • Computer skills that include Microsoft Office, Word, Excel and Outlook and PowerPoint.
  • Excellent presentation and communication skills.
  • Clinical Background with ability to conduct Patient Assessments.
  • Ability to work under stress and to respond quickly in crisis situations.
  • Knowledge of healthcare marketing and plan development.
  • Ability to work cooperatively as a member of a team.
  • Must read, write and speak fluent English.
  • Must have good and regular attendance.
  • Associate or Bachelor’s degree.
  • Two (2) years clinical patient care/treatment experience in healthcare field preferred.
  • Demonstrated understanding of Medicare, Medicaid, and other payers as related to LTACH benefits.

Nice To Haves

  • Degree in healthcare related field preferred.
  • Healthcare professional licensure preferred.
  • Registered Nurse, Licensed Practical Nurse, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Social Worker license.

Responsibilities

  • Conduct patient assessment in accordance with company standards.
  • Work closely with patients and their family members to educate them on the benefits of Kindred Hospital.
  • Assist in conducting tours of the hospital with families and referral sources as needed.
  • Facilitates the timely admission of patients from the referral sources: Identifies medical record information needed from referring facility; Completes the Pre-Admission Clinical Evaluation (PACE) form to assist in evaluating appropriateness of admission; Facilitates and coordinates physician-to-physician communication as necessary; Identifies specific patient needs for equipment, supplies and consult services.
  • Meets with management staff, participates in marketing meetings and serves on various Internal committees as assigned.
  • Maintains a state-of-the-art knowledge of areas of responsibility and develops and follows a program of continuing education.
  • Conducts job responsibilities in accordance with the standards set out in the Company’s Code of Business Conduct, its policies and procedures, the Corporate Compliance Agreement, applicable federal and state laws, and applicable professional standards.
  • Maintains all information in a manner that ensures confidentiality and meets all regulatory requirements.
  • Performs other related duties as assigned.
