About The Position

The Passport Program is a reimbursement funding program for adults within Ontario, to access services and supports within their communities. The purpose of the position is to support the increase of the utilization of the Passport Program, ensuring existing clients are aware of and able to use their assigned funding to access their community. This role involves observing service issues, identifying trends in funding not being utilized, and reporting client concerns to the Passport team.

Requirements

  • Be aged between 15 and 30 years old on the start date of the job
  • Have a valid Social Insurance Number (SIN)
  • Be a Canadian citizen, a permanent resident of Canada, or a person who has been granted refugee status in Canada

Responsibilities

  • Work with Supervisor and staff to prepare an up-to-date utilization report
  • Assist in the contact and review of information related to the program
  • Assist with client information about funding amounts and how to access it
  • Work with Supervisor and staff to identify trends, complaints regarding communication within the program (e.g., mail out and email correspondence)
  • Assist with coordinating meeting times with Direct Funding worker, to address any ongoing concerns
  • Maintain utilization report for accurate and current information related to client feedback
  • Ensure client contacts is up to date
  • Provide support for additional research and program tasks, as assigned
  • Must be aware of and adhere to Health & Safety Policy Rules & Regulations
  • To adhere to all agency standards, policies and procedures demonstrating knowledge and skill in decision making and problem solving and participating as a team member
  • Provide a welcoming, professional working environment, respecting the values, culture, and beliefs of staff, persons served, and their families
