About The Position

The Participant Care Associate (CNA) provides direct personal care and support to older adults enrolled in the Program of All-Inclusive Care for the Elderly (PACE) at Hudson Headwaters in Glens Falls, New York. The role helps participants maintain safety, dignity, independence, and quality of life while working with an interdisciplinary healthcare team.

This is a full-time, Monday–Friday position scheduled from 8:00 AM to 4:30 PM, with no weekends required. The stated pay is $22.50 per hour; the posting also includes a pay notice of $23–$23 an hour.

Requirements

• Active New York State CNA Certification.
• High school diploma or GED.
• Ability to provide hands-on personal care and work with elderly and frail populations.
• Experience or comfort assisting with bathing, grooming, dressing, toileting, feeding, transfers, and ambulation.
• Ability to work Monday–Friday, 8:00 AM–4:30 PM.
• Ability to lift up to 50 lbs occasionally.
• Reliable transportation.
• Ability to complete medical clearance and background checks.
• Strong communication and documentation skills.
• Ability to work within an interdisciplinary healthcare team.

Responsibilities

• Provide hands-on personal care, including bathing, grooming, dressing, and toileting.
• Assist with feeding and meal support.
• Assist participants with transfers and ambulation.
• Monitor participant well-being and report concerns.
• Complete participant care documentation.
• Support participant independence and safety.
• Follow individualized care plans and collaborate with interdisciplinary team members.
• Maintain participant dignity and comfort.
