About The Position

The Home Care Participant Care Assistant (CNA) provides essential in-home care and assistance to participants enrolled in the PACE program. Working closely with the Home Care Supervisor, the assistant delivers compassionate, person-centered care and supports activities of daily living. The work includes assisting with daily activities and providing comfort and companionship to participants.

This is a full-time role scheduled for 40 hours per week, 8:00 a.m. to 4:30 p.m., Monday through Friday. The role involves regular travel to provide home-based care in the Niles, Michigan area.

Requirements

• One year of prior experience with frail or elderly individuals, assisting with activities of daily living.
• Valid Michigan driver’s license, clean driving record, and reliable transportation.
• CNA certification.
• Current CPR or BLS certification, or willingness to obtain it.
• Ability to work independently, multitask, and work as part of an interdisciplinary team.

Responsibilities

• Assist participants with personal care and activities of daily living, including bathing, grooming, dressing, and toileting.
• Help participants with meals and hydration.
• Support mobility, including turning, positioning, and safe transfers.
• Measure and record vital signs, weight, and intake and output.
• Assist with housekeeping tasks such as laundry, sweeping, and grocery shopping.
• Provide responsive care with kindness and compassion.
• Report changes in participants’ conditions to leadership promptly.
• Maintain confidentiality and demonstrate respect and professionalism.
