About The Position

The Advanced Community Care Clinician (ACCC/AC3) delivers medical care in the communities where Cityblock members live, including homes, shelters, offices, public places, and streets. The role primarily facilitates virtual provider visits with members and follows medical orders, performing clinical skills and tasks within the clinician’s scope, training, and policy. Depending on local legal regulations, these may include IV insertion, IV/IM/SQ medication administration, 12-lead EKGs, and phlebotomy. The clinician connects members to Cityblock services, including medical providers and other available services. Visits may include acute illness visits at home facilitated by a provider, facilitated visits with a longitudinal care team provider, independent outreach to hard-to-reach members, specimen collection for laboratory testing, and MIC-only follow-up visits with specific goals. Each shift begins with reviewing equipment, communications, and scheduled visits. The position is based in Suffolk County, Massachusetts, with occasional work outside the county to provide backup or meet unexpected needs. Approximately 95% of visits take place in members’ homes; other visits occur wherever members are comfortable. The schedule is either Monday–Friday, 8:45 AM–5:15 PM, or four days per week, 8:45 AM–7:15 PM, with workdays determined by team needs. A typical day includes 3–4 member visits.
