About The Position

This is an hourly, part-time position for a Clinical Social Worker in Inpatient Case Management. Trinity Health is a leading healthcare provider in Southeastern Michigan, with Trinity Health Livonia being a 304-bed acute care hospital. The Social Worker will provide services to patients, families, and significant others to help them cope with adjustment to illness, understand diagnoses, prognoses, and treatment. A key responsibility is to implement a safe and appropriate transition to the next level of care. The role involves working closely with the Palliative Care team, including NP, Physician, and Chaplain, to follow up on palliative care consults.

Requirements

MSW Masters Degree in Social Work from an accredited school of social work
State of Michigan license, LMSW, or license eligible, LLMSW
2 years as an MSW Social Worker in an acute care setting

Responsibilities

Assess patient/family social and emotional factors to estimate their potential to cope with terminal illness and death.
Provide psycho-social counseling and support to patients/families experiencing emotional, social, and economic conflict.
Assist patients/families in utilizing community resources and community PC programs.
Participate in interdisciplinary team meetings to establish, review, and revise individualized plans of care.
Discuss patient-centered goals of care, disease prognosis, and advanced care planning.
Support the patient's unique spiritual and cultural beliefs.
Participate in developing and implementing appropriate measures of patient care quality and satisfaction.
Perform ad hoc activities to support educational and outreach goals of the palliative care service.
Provide critical psycho-social support to Palliative Care patients through the palliative care services consult team.
Support the inpatient case manager/social worker by developing a plan of care for patients seen by the Palliative Care service.
Complete referrals for patients going into GIP hospice under the Palliative Care service, in coordination with inpatient social workers and CM.
Use and develop the social work process of assessment, planning, implementation, and evaluation.
Collect and assess data related to individual healthcare needs, including past medical history, current health status and treatment, necessity/appropriateness for services and setting, and follow-up needed.
Reassess on a consistent basis and document in the EMR, including changes in patient condition, goals, preferences, and as defined by the palliative care program.
Provide counseling to enable patient/family members and/or significant others to utilize resources and strengths to make necessary adjustments due to illness, its impact, and anticipated changes.
Include patient and family and/or support person in decision-making.
Actively participate in multidisciplinary PC team rounds and communicate with the Case Management team/physicians information that guides the plan of care.
Assist in referrals for bereavement resources for family and/or support person prior to patient's death.
Identify the need for and convene family meetings to provide clarity to the patient’s status that result in informed decision making.
Be familiar with legal requirements regarding reporting alleged child and/or adult neglect or abuse.
Assess situations to determine if a report needs to be made to Children’s Protective Services or Adult Protective Services.
Coordinate with the physician, nurse manager, and case manager to implement a safe plan of care for a child if there are concerns regarding abuse/neglect.
Coordinate with the physician, nurse manager, and case manager to implement a safe plan of care for an adult.
Coordinate with the domestic violence service agency to plan counseling service and/or a shelter placement, if necessary.
Document in patient’s electronic database ongoing communication with all individuals responsible for the continuum of care or regulatory issues.
Demonstrate the ability to use the various computer programs required.
Conduct support/education groups to assist in the adjustment process.
Handle community calls and visits if needed.
May rotate on weekends and holidays as needed in the department.
