About The Position

The Palliative Care Program Coordinator is a highly skilled professional responsible for providing palliative care consultations and coordinating the growth and development of the palliative care program. This role involves leading, educating, and mentoring Palliative Care Providers within the organizations, collaborating with leadership on program goals, education, and development. The coordinator will stay updated on regulatory and practice changes, participate in staff and community education, and collaborate with the palliative care team on care interventions. They will also contribute to the review and development of practice protocols, quality assessment, and performance improvement initiatives. Additionally, the role involves advancing hospice and palliative care through professional involvement, maintaining a patient panel, conducting consultations, and communicating outcomes to providers and referral sources. Responsibilities include documentation of patient care, managing pain and symptom changes, resolving conflicts, and participating in research or special projects as assigned.

Requirements

• Minimum of five (5) years of professional experience in health care.
• Experience in a hospice care required.
• Experience in palliative care required, preferably 2+ years.
• A master’s or doctoral degree in nursing or physician assistant studies.
• Current CPR Certification.
• Current Massachusetts licensure: Nurse Practitioner or Physician Assistant.
• Valid driver’s license.
• Hospice and Palliative Care Nurse Certification required.

Nice To Haves

• Experience in homecare preferred.
• Understandings of hospice and palliative care philosophy, and issues of death/dying, goal concordant prescribing and serious illness conversations and goals of care establishment.
• Experience in operational care delivery coordination.
• Demonstrates good verbal and written communication, and organization skills.
• Strong observation skills and problem-solving skills.
• Experience working with an Interdisciplinary Team preferred.

Responsibilities

• Collaborate with leadership regarding program goal planning, education, and program development and growth by maintaining up to date knowledge of regulatory and practice changes as they relate to the provision of palliative care.
• Participate in planning and provision of ongoing education of agency staff and community partners through workshops, in-services, and bedside consultation.
• Collaborate with the Palliative Care team on appropriate care interventions as needed.
• Participate in the review and development of practice protocols by proposing interventions consistent with palliative care literature and research, including the development of quality assessment and performance improvement related to palliative care.
• Participate in program and professional development within the adult palliative care department, including orientation and education of staff and stakeholders.
• Participate in quality performance improvement projects to drive high-quality care in the adult palliative care department.
• Participate in the advancement of Hospice & Palliative care through involvement with local, state, and national trade organizations.
• Maintain a regular panel of palliative care patients providing consultations and collaborating with primary and specialty care providers.
• Responsible for contacting assigned patient and/or family to arrange a meeting to provide information to the prospective patient and/or family and/or referring professional.
• Complete Hospice Face-to-Face visits as needed with an assessment of the patient to be provided to the Medical Director to aid in determination of eligibility for hospice services.
• Responsible for informing the primary providers and referral sources the outcome of the Nurse Practitioner consultations and evaluations via recommendation and collaboration.
• Maintain regular contact with referral sources as needed.
• Complete, maintain and submit accurate and relevant clinical notes regarding patient’s condition and care. Record pain and symptom management changes and/or outcomes as appropriate.
• Complete and submit Point of Service documentation within 24 hours of consultation.
• Participate in the resolution of interpersonal conflict and issues of clinical and ethical concern as needed with palliative care patients, families and/or caregivers.
• Participate in research activities or special projects as assigned and needed.
• Perform other related duties as assigned.
