About The Position

Central Coast Home Health is seeking a Full-time Palliative Care Case Manager to join their team. This role serves patients with life-limiting diseases who are still undergoing curative treatments. The focus is on symptom management, advance directives, end-of-life decisions, and patient/family education. The Palliative Care Nurse collaborates with a multidisciplinary team, including social workers, spiritual counselors, and therapists. The position involves conducting home visits to build relationships, address patient and family needs on a psychosocial, physical, and spiritual level.

Requirements

Current California RN License
At least one year of professional nursing experience
CPR certified (Healthcare Provider)
Reliable vehicle to drive for work
Current Driver's license and clean driving record
Maintain auto insurance coverage
Comfortable working independently in a home-visit setting
Able to effectively communicate with elderly and chronically ill patients and families.

Nice To Haves

Home Health and/or Hospice experience is a plus.

Responsibilities
