About The Position

Central Coast Home Health is seeking a Full-time Palliative Care Case Manager to join their team. This role serves patients with life-limiting diseases who are still undergoing curative treatments. The primary focus is on symptom management, advance directives, end-of-life decisions, and patient/family education about disease processes. The Palliative Care Nurse collaborates with a multidisciplinary team, including social workers, spiritual counselors, and therapists. By conducting home visits, the nurse develops relationships, addresses patient and family needs on psychosocial, physical, and spiritual levels, and provides necessary education.

Requirements

Current California RN License
At least one year of professional nursing experience
CPR certified (Healthcare Provider)
Reliable vehicle to drive for work
Current Driver's license and clean driving record
Have and maintain auto insurance coverage
Comfortable working independently in a home-visit setting
Must be able to effectively communicate with elderly and chronically ill patients and families.

Nice To Haves

Home Health and/or Hospice experience is a plus
