About The Position

Central Coast Home Health is seeking a Full-time Palliative Care Case Manager to join their growing team. This role focuses on caring for patients with life-limiting diseases who are still undergoing curative treatments. The Palliative Care Nurse will concentrate on symptom management, advance directives, end-of-life decisions, and patient/family education regarding disease processes. The nurse will collaborate with a multidisciplinary team, including social workers, spiritual counselors, and therapists. By conducting home visits, the nurse will build relationships, address patient and family needs on psychosocial, physical, and spiritual levels, and provide necessary education.

Requirements

  • Current California RN License
  • At least one year of professional nursing experience
  • CPR certified (Healthcare Provider)
  • Reliable vehicle to drive for work
  • Current Driver's license and clean driving record
  • Maintain auto insurance coverage
  • Comfortable working independently in a home-visit setting
  • Must be able to effectively communicate with elderly and chronically ill patients and families.

Responsibilities
