About The Position

We are looking for a Palliative Care Navigator to provide services to patients and those supporting them who are navigating the complex healthcare continuum. The Palliative Care Navigator works directly with patients (or with their legal representatives) to ensure they have a voice in their care and information to promote informed decision making. The Palliative Care Navigator serves as an educator to internal and external key stakeholders. This role involves providing services similar to hospice licensed practical nurses (LPNs) who are committed to creating meaningful patient experiences. As a hospice licensed practical nurse (LPN), you’ll work under the direction and supervision of the registered nurse to conduct patient visits and provide routine care, while communicating with the care team, the patient and their family. Requirements

  • A heart to serve patients and families and a passion for providing the best possible care
  • Graduate of an accredited program for Practical Nurse or Vocational Nurse.
  • Minimum 3 years of experience required in General Nursing or Medical Assistance.
  • Reliable transportation.
  • Valid Driver's License and Insurance.

Nice To Haves

  • Current home health, hospice, medical, surgical or critical care experience preferred.
