LPN, Palliative Care Coordinator

Requirements
- Must possess a current, unencumbered LPN License
- Experience in scheduling in a healthcare organization or unit
- Adept knowledge of the Palliative Care program and ability to explain to the fullest extent palliative care utilization and services available.
Responsibilities
- Receive and promptly process palliative care referrals and/or inquiries for services provided by Crossroads.
- Work in cooperation with the call center for incoming or received referrals.
- Maintain accurate and detailed records of all information calls and referrals.
- Schedule and assign all palliative patients with designated Palliative Care Nurse Practitioners as appropriate.
