About The Position

At Guthrie Robert Packer Hospital, you'll join an interdisciplinary Palliative Care team dedicated to improving quality of life for patients facing serious illness. Working closely with physicians, advanced practice providers, social workers, patients, and families, you'll serve as a guide, educator, and advocate throughout the patient journey. As a Palliative Care Nurse Navigator, you'll help support patients and families throughout their healthcare journey. Responsibilities include coordinating care between inpatient and outpatient settings, participating in hospital rounds, conducting patient follow-up, facilitating transitions to hospice services when appropriate, providing education to patients and families, and serving as a resource for providers and clinical staff. This role combines the relationship-centered aspects of traditional nurse navigation with the unique opportunity to support patients across multiple care settings, ensuring continuity, communication, and compassionate care throughout the treatment process. Requirements

  • Diploma from an accredited school/college of nursing
  • RN licensure at time of hire.

Responsibilities

  • Coordinating care between inpatient and outpatient settings
  • Participating in hospital rounds
  • Conducting patient follow-up
  • Facilitating transitions to hospice services when appropriate
  • Providing education to patients and families
  • Serving as a resource for providers and clinical staff
  • Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
  • Coordinates efforts for treatment and promotes quality improvement and educational efforts.
  • Compiles data, tracks outcomes, and makes recommendations for process improvement.
  • Provides referrals/support for follow-up care for patients who are uninsured or underinsured.
