Working closely with physicians, advanced practice providers, social workers, patients, and families, you'll serve as a guide, educator, and advocate throughout the patient journey. As a Palliative Care Nurse Navigator, you'll help support patients and families throughout their healthcare journey. Responsibilities include coordinating care between inpatient and outpatient settings, participating in hospital rounds, conducting patient follow-up, facilitating transitions to hospice services when appropriate, providing education to patients and families, and serving as a resource for providers and clinical staff. This role combines the relationship-centered aspects of traditional nurse navigation with the unique opportunity to support patients across multiple care settings, ensuring continuity, communication, and compassionate care throughout the treatment process.
