About The Position

Under the general guidance of the Executive Director of APCs and with medical oversight from MIH Physician leadership, the Physician Assistant or Nurse Practitioner provides telehealth care to patients in the community. This rapidly growing service line is adding provider coverage to meet community needs. The provider evaluates, diagnoses, and treats patients in accordance with written guidelines approved by the Department of Medicine.

Requirements

• Current ACLS (AHA) certification, including courses offered through South Shore Health.
• Physician Assistant license from the Massachusetts Board of Registration of Physician Assistants.

Responsibilities

• Provide telehealth services to patients and community partners for COVID testing and discharge follow-ups after ED or hospital care for COVID symptoms.
• Conduct telehealth evaluations and coordinate community-based testing and incoming requests from skilled nursing facilities, assisted living facilities, and psychiatric hospitals.
• Complete provider-to-provider screenings and order testing for symptomatic, home-bound, and at-risk populations.
• Document provider-to-provider consultations and telehealth visits in the SSH MIH Epic code, and coordinate with Registration to update patient information.
• Coordinate with EMS Dispatch to print lab labels and dispatch MIH/EMS to collect specimens.
• Review Home Monitoring Telehealth Reports and contact discharged patients to conduct telehealth follow-up visits.
• Determine and coordinate next steps following telehealth visits, including program discharge and device collection, continued monitoring, MIH paramedic visits, PCP follow-up, VNA services, or MIH Medical Control oversight.
• Consult with MIH paramedics by telehealth when additional consultation is required and determine appropriate follow-up actions.
• Coordinate orders and services with MIH Administration, PCPs, VNA, and MIH Medical Control as needed.
• Participate in continued learning and learn and use new technology and procedures developed for the role and organization.
• Embrace technological advances that support effective and efficient communication.
