About The Position

EMS Unlimited is preparing to launch a Community Paramedic / Mobile Integrated Health program in Garfield County, with patient care expected to begin in early 2027. The company is seeking a full-time Community Paramedic to join before launch and help build the program. The role works from a dedicated CP vehicle, visiting enrolled patients in their homes and collaborating with their physicians to reduce care gaps that can lead to emergency department visits and readmissions. Practice is under Colorado’s Community Paramedic endorsement and CP protocols, with real-time telehealth access to a supervising physician. Patients may include people recently discharged and at high risk of readmission; frequent emergency department users or people without stable primary care; patients managing chronic disease at home; older adults at risk of falls; and people with behavioral health or substance use needs who require follow-up and connection to services. The Community Paramedic provides in-home assessment and clinical care, post-discharge transitional care, chronic disease education, safety and social-needs screening, care coordination, and documentation. The position also contributes to building CP protocols, referral workflows, and monthly quality reviews. The schedule is Monday through Friday on day shift, with exact hours still being designed. Travel throughout Garfield County during the shift is in a company vehicle.
