PT - Outreach Worker (Homeless Prevention, Vancouver)

About The Position

This position will provide intensive supports and services to eligible individuals through the Homelessness Prevention Program (HPP), funded by BC Housing, facilitating access to supports services that will assist in meeting their housing needs.

Requirements

  • Diploma and/or a degree in social or behavioural sciences
  • One year’s experience working with individuals who are unhoused or other housing related experience
  • Experience supporting individuals facing multiple barriers to community inclusion due to substance use, mental health concerns, criminal legal involvement, experiences of violence or oppression, or other challenges.
  • Experienced with harm reduction approaches and willingness to apply these approaches
  • Demonstrated and clear ability to respond well in crisis situations
  • Extensive knowledge of community resources
  • Experience in assessment, case plan development and implementation
  • Demonstrated ability to develop programs tailored to the particular diverse needs of the individual
  • Excellent written and verbal communication skills in English
  • BC Class 5 driver's license
  • Personal transportation that is in good repair, insured for business up to $1,000,000 for liability, and which can be used to transport service users
  • Ability to successfully complete a Criminal Records Check
  • Ability to successfully complete reference check
  • Current WorkSafe Approved Emergency First Aid Certificate

Responsibilities

  • Receives and reviews referrals from Connective’s Community Services Office and community partners to determine eligibility and suitability for the program
  • Provides outreach, case management, advocacy, and other needed services to participants in any environment including streets, shelters, hospitals, institutions, apartments, and the drop-in office
  • Works with each individual to create a mutually agreed intervention and support plan that has measurable short and long-term goals
  • Ensures issues of daily living, including but not limited to housing, basic needs, hygiene, leisure, employment, and education are addressed in this plan
  • Supports and provides services for individuals who are homeless, or at risk of homelessness, and have lived experiences of substance use, mental health, involvement in the criminal justice system, intimate partner violence, and trauma
  • Provides a wide range of interventions to enable each service user to address and accomplish the established goals
  • Interacts with other professionals, family members and organizations to ensure services are in the service user’s interest and supportive of the long-term goals
  • Maintains written and computerized records, compiles reports and completes other program documentation (including case notes, statistics, letters, available file information, etc.) in accordance with the policy and procedures manuals of the society and in accordance with funder requirements
  • Initiates, coordinates and monitors referrals to community services, and advocates for service users’ participation in them
  • Creates, builds, and fosters partnerships with housing providers and landlords
  • Educates, informs and advocates for participants regarding benefits and entitlements (social assistance, rental subsidies, employment supports, low income services i.e. food bank and food stamps)
  • Participates in staff meetings and meetings with funders and community partners as instructed by the Program Manager
  • Other duties as subsequently negotiated
