POSITION SUMMARY The Homeless Prevention Specialist is responsible for providing comprehensive case management and supportive services to individuals and families who are at risk of homelessness or currently experiencing housing instability. This position focuses on housing stabilization, resource coordination, client advocacy, and ongoing support to help clients achieve self-sufficiency and maintain stable housing. The specialist will maintain a caseload of Psychiatric Rehabilitation Program (PRP) clients, facilitate group sessions, and assist with program authorizations as needed.

Requirements

  • Strong organizational, communication, and documentation skills.
  • Ability to work independently and as part of a multidisciplinary team.

Nice To Haves

  • Bachelor’s degree in Social Work, Human Services, Psychology, Counseling, or related field preferred.
  • Experience working with homeless or vulnerable populations preferred.
  • Experience with PRP services, behavioral health services, or case management preferred.

Responsibilities

  • Maintain an assigned caseload of PRP clients and provide ongoing PRP services.
  • Complete assessments, service plans, progress notes, and required documentation.
  • Develop individualized housing stabilization plans.
  • Assist clients with securing and maintaining housing.
  • Coordinate referrals and community resources.
  • Conduct in-person group sessions at shelter locations and the Mettle Works home office.
  • Facilitate groups focused on housing stability, life skills, financial literacy, employment readiness, wellness, and recovery.
  • Assist with PRP and PHP authorizations and reauthorizations.
  • Maintain accurate and timely records in compliance with agency requirements.
  • Participate in case reviews, team meetings, and quality assurance activities.
  • Establish relationships with shelters, housing providers, and community organizations.
  • Perform additional tasks and duties as needed.
