About The Position

Coordinates and identifies community members needing information about HIV testing, treatment and prevention and Sexually Transmitted infection testing, treatment and prevention. Works with community agencies, does HIV/STI testing in a mobile van. Schedules patients for initial appointments. Works in the community and in the outreach office. Provides health education and resources. Contacts and schedules patient/client’s appointments for consultations, meetings with community agencies and/or diagnostic tests. Sets-up and maintains files and records. Collects and organizes statistical reports.

Requirements

  • High School Diploma or equivalent required.
  • 0-1 years of relevant experience, required.
  • Ability to communicate effectively while being sensitive to cultural diversity.

Nice To Haves

  • Additional language skills are appreciated but not required.
  • Previous experience in HIV / HIV prevention case management, preferred.
  • Bilingual - fluent in Spanish, preferred.
  • Prior experience with the LGBTQ+ community, preferred.

Responsibilities

  • Contacts patients; schedules appointments for consultations, agency meetings and/or diagnostic tests.
  • Collects and organizes data required for statistical reports and completes reports, as required.
  • Attends and participates in meetings as appropriate.
  • Sets-up files and maintains records.
  • Provides incoming and in-process orientation.
  • Based on position, may be required to provide bi-lingual translation and/or guidance.
