About The Position

Are you ready to make a meaningful impact in the community? Join us as an Outreach LPN in Sauk Centre and work alongside dedicated providers for various outreach clinics. The LPN will provide patient rooming for all types of clinic visits, communicate results and other pertinent medical information to patients and providers, assist in assuring the day-to-day activities of the clinic are completed efficiently and assist providers with clinic procedures among various other duties.

Requirements

Graduate from an accredited school of nursing
Licensed Practical Nurse (LPN)
Current licensure in the State of Minnesota
Basic Life Support (BLS)
Current American Heart Association (AHA) BLS Healthcare Provider card per established policy
Must function independently, have flexibility, personal integrity and ability to work effectively with patients, families, medical staff and general public

Nice To Haves

To be eligible for the hiring and relocation bonus, you must be an external candidate who is hired into an eligible full-time or part-time benefited position. Previous employees qualify if they have not worked for CentraCare for at least 9 months at the date of hire. Relocation assistance also requires that you currently reside more than 50 miles away and relocate to within 30 miles of your primary work location within 120 days.
