About The Position

The Outreach and Intake Specialist under the direction of the Program Manager, ensures delivery of services to aide in job placement by promoting and developing awareness of the program and conducts initial eligibility by pre-screening for the Homeless Veteran Reintegration Program (HVRP). This role involves developing effective, trusting relationships with program participants using a trauma-informed care and strength-based approach, modeling positive interactions, and conducting community outreach to engage participants. The specialist will also conduct initial eligibility screenings, build a database of community resources, and advocate for eligible participants while providing referrals for those who do not meet eligibility requirements. A key part of the role is completing intakes, evaluating needs, and setting up client files for Case Managers. Additionally, the specialist will develop and maintain relationships with employers and community partners to assist participants in obtaining and maintaining employment, commit employers to assisting veterans, and conduct outreach and networking activities, including giving presentations. The role requires active participation and networking in monthly community meetings to promote the success of the Homeless Veterans Reintegration Program and maintain a team calendar of events. Documentation is crucial, including weekly or daily case notes, comprehensive participant files, reports on metrics, and data entry into Salesforce and VGRS systems. Ongoing communication with multi-disciplinary teams is necessary to help participants secure and maintain housing and stability. Follow-up with program exits is also part of the role. Travel is required between Escambia, Santa Rosa, and Okaloosa Counties. The position emphasizes exceptional customer service to support the mission of empowering people to build better lives.

Requirements

  • Bachelor's Degree in Social Work, or Public Administration, or Psychology or Human Services or related field preferred or equivalent in years of experience.
  • 1+ years of experience is a social service agency
  • 1+ years in a customer service focused environment.
  • 1+ years working knowledge with Veteran Community.
  • Experience with mental health services and/or a homeless population.
  • Must maintain regular and acceptable attendance.
  • Driver's License with clear record.
  • Must have daily use of a vehicle without prior notice.
  • Must be available and willing to travel to various locations and with such frequency as the business need dictates.
  • Must be available and willing to work nights, weekends and holidays as required to meet business needs.
  • Must not pose a direct threat or significant risk of substantial harm to the safety or health of himself/herself or others.

Responsibilities

  • Develops effective, trusting relationships with program participants by utilizing trauma informed care and strength base approach.
  • Models positive interactions, verbal guidance, and effective communication.
  • Conducts Community outreach to engage and bring participants into the program.
  • Conducts initial eligibility screening for individuals seeking program assistance.
  • Builds database of community resources.
  • Advocates for eligible participants, provide general information and community referrals for those not meeting program eligibility requirements.
  • Completes Intake for eligible participants by evaluating individual and/or family needs.
  • Assists in setting up initial client file for clients being assigned to a Case Manager.
  • Develops and maintains comprehensive outreach/working relationship with employers, recruiting teams, and community partners on the participants’ behalf to develop workable plans to obtain and/or maintain employment.
  • Have employers and partners commit to assisting veterans.
  • Conducts outreach and networking activities in the community.
  • Gives presentations at various community groups/events.
  • Remains active and networks in monthly community meetings.
  • Promotes the success and reputation of the Homeless Veterans Reintegration Program.
  • Seeks out and maintains HVRP Team calendar with all relevant community events, speaking presentations, partnerships meetings.
  • Documents weekly or daily case notes; submits comprehensive participant files, completes reports as needed, and enters documentation into Salesforce and VGRS systems.
  • Prepares necessary reports on metrics.
  • Maintains on-going communication with multi-disciplinary teams across multiple programs to assist participants in obtaining and maintaining permanent housing, services to meet needs, and promote stability.
  • Follow-up with participants who have exited the program and provide further assistance or resources, if needed.
  • Travel between Escambia, Santa Rosa and Okaloosa Counties to include, but not limited too, the above job duties as listed above.
