About The Position

Make a Difference Every Day Ray Graham Association (RGA) is a leader in supporting people with intellectual and developmental disabilities to live meaningful, self-directed lives. We are seeking a compassionate, organized, and customer-focused Service Intake and Outreach Coordinator to serve as the first point of contact for individuals, families, referral sources, and community partners seeking services. The Service Intake and Outreach Coordinator plays a vital role in connecting individuals with the supports they need by coordinating referrals, navigating service options, maintaining intake processes, and collaborating across programs to ensure a positive experience for those seeking services.

Requirements

  • High school diploma or GED required.
  • Valid driver's license with an acceptable driving record.
  • Minimum of two years of experience in intake, customer service, case management, healthcare, human services, or a related field.
  • Excellent verbal and written communication skills.
  • Strong organizational, documentation, and time-management skills.
  • Ability to handle multiple priorities while maintaining attention to detail.
  • Proficiency with Microsoft Office and computerized database systems.
  • Ability to work collaboratively with diverse individuals, families, and community partners.
  • Must be able to maintain reliable transportation as required for business needs.

Nice To Haves

  • Bachelor's degree in Human Services, Social Work, Healthcare Administration, Psychology, Special Education, or a related field.
  • Experience working with individuals with intellectual and developmental disabilities.
  • Knowledge of the Illinois DHS Division of Developmental Disabilities system, PUNS, Medicaid Waiver programs, and Independent Service Coordination (ISC) processes.
  • Experience with referral management, service coordination, or admissions processes.
  • Bilingual skills are a plus.

Responsibilities

  • Serve as the primary contact for inquiries regarding RGA services and supports.
  • Respond to referrals from individuals, families, hospitals, schools, community agencies, and other referral sources.
  • Conduct intake screenings and collect necessary documentation.
  • Guide individuals and families through the admissions and enrollment process.
  • Maintain accurate records of referrals, vacancies, and intake activities in organizational databases.
  • Coordinate communication among internal departments to facilitate service placements.
  • Assist individuals and families in navigating the Illinois intellectual and developmental disabilities (I/DD) service system, including available programs and resources.
  • Develop and maintain positive relationships with Independent Service Coordination (ISC) agencies, state agencies, healthcare providers, and community partners.
  • Prepare reports and monitor referral and intake data to support organizational planning and service delivery.
  • Participate in outreach, advocacy, and community engagement activities to increase awareness of RGA services.
  • Ensure all interactions are person-centered, respectful, and aligned with RGA's mission and values.
