Adult Community Clinical Services is a Department of Mental Health-funded service that delivers evidence-based interventions within a clinically focused model. The people served have mental illness or a dual diagnosis and live in their own homes, apartments, or group living environments. Services help people transition to more independent environments, maximize natural supports, build skills, and achieve personal goals. Teams are multidisciplinary and include clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff. Outreach Counselors liaise among people served, community agencies, and families to coordinate treatment, including with employment and education entities, emergency service providers, and outpatient clinics. They cultivate mutually respectful partnerships, set appropriate limits, and maintain a helping role with individuals and family members. They encourage and empower people to move beyond perceived limitations by providing tools for success. The role delivers Treatment Plan interventions under the supervision of licensed clinical staff, supports opportunities for valued community roles and relationships, and provides side-by-side coaching for socialization. Counselors conduct individual and group activities to increase readiness for rehabilitation, help people obtain financial support through employment or entitlements, and participate in the on-call rotation.
