About The Position

The Center for Human Development (CHD) is seeking an Outreach Caseworker to join their team. This role provides direct services and casework support to program participants with developmental disabilities who live independently in the greater Westfield/West Springfield and Springfield community. The position involves supporting clients with Traumatic Brain Injury and other program participants who live independently in Hampden/Hampshire county. Responsibilities include assisting participants in achieving their goals, managing health needs, and developing skills for independent living to optimize their independence. The role also involves helping individuals obtain and maintain entitlements, manage funds, and develop social and recreational experiences. Additionally, the Outreach Caseworker will provide written documentation and assist the Case Manager with Individual Service Plan development, assessments, and progress updates.

Requirements

  • Bachelor's Degree or equivalent experience
  • Strong interpersonal skills
  • Empathy for others
  • Demonstrated commitment to serving those with significant needs
  • Ability to work well within a team model and independently
  • Computer skills necessary
  • Good writing and communication skills
  • Excellent interpersonal skills
  • At least 21 years of age
  • Must have a vehicle for work use
  • Current driver's license
  • Willingness to transport program participants in own vehicle
  • Must complete and pass background record screening process
  • Must complete and pass a driving record check

Responsibilities

  • Provide direct services to clients with Traumatic Brain Injury
  • Provide casework supports to program participants who live independently
  • Support program participants to achieve their individual goals and objectives
  • Manage program participants' ongoing health maintenance needs
  • Provide supports/skill building for all aspects of independent living
  • Provide opportunities to optimize program participants' independence
  • Help people to obtain and maintain all generic entitlements
  • Responsibly manage program participants' funds
  • Develop rewarding social and recreational experiences for program participants where desired
  • Provide and maintain written documentation
  • Assist Case Manager with Individual Service Plan development, assessments, and progress updates
