About The Position

Outpatient Registered Nurse Heart of Hopi Country – Northeastern Arizona The Community – Polacca, AZ Polacca sits at the base of First Mesa on the Hopi Reservation in northeastern Arizona, surrounded by the dramatic mesas, buttes, and high-desert landscapes of the Colorado Plateau. About the Facility – Hopi Health Care Center (HHCC) Hopi Health Care Center is a Joint Commission-accredited Critical Access Hospital and the only healthcare facility on the Hopi Reservation, serving approximately 18,000 people. HHCC provides 24/7 Emergency Department services, a four-bed short-stay inpatient unit, and business-day ambulatory services including medical, dental, physical therapy, optometry, and public health. Requirements

• Graduate of an accredited nursing program (BSN preferred).
• Current, active, full, and unrestricted RN license in any U.S. state or territory.
• Minimum 1–2 years of outpatient, ambulatory, or clinic nursing experience preferred.
• Current BLS certification required.

Nice To Haves

• Rural/remote, Critical Access Hospital, or IHS/Tribal healthcare experience preferred.

Responsibilities

• Provide direct nursing care in the HHCC outpatient/ambulatory clinic setting.
• Perform patient assessments, triage, care coordination, medication administration, and patient education.
• Assist providers with examinations, procedures, and treatment plans.
• Coordinate referrals, follow-up appointments, and care transitions.
• Administer immunizations, manage chronic disease follow-up, and support public health programs.
• Document all clinical encounters in the EHR per facility policies.
