About The Position

PRN RD LD position open with an established outpatient nutrition program with a wide variety of diagnoses. The Diabetes Care Clinic helps patients manage their condition and enabling them to lead the lives they want to lead. The program offers Plantar Fasciitis (PF) preventive and maintenance services for adults suffering from diabetes. Under limited supervision, provides professional medical nutrition therapy to groups and individuals of all ages and abilities, in a variety of settings in the outpatient nutrition service area. Follows established standards of practice and guidelines defined by the Academy of Nutrition and Dietetics, USDA, American Diabetes Association, and other evidenced based resources for the assessment of patients, and subsequent planning, implementation, evaluation of nutrition education. Services also includes continuum of care for out-patients requiring medical nutrition therapy. Works with an established multidisciplinary team of professionals to provide quality nutrition education services including all department services. Also provides support the outpatient diabetes education services as needed.

Requirements

  • Bachelor’s Degree
  • Current registration or registration-eligible with the Commission on Dietetic Registration. Those registration-eligible must become registered within 6 months of date of hire unless extension granted
  • Licensed Dietitian in Kentucky or licensed-eligible after receiving CDR registration. Those licensed-eligible must become licensed in KY within 6 months after receiving CDR registration unless extension granted.

Responsibilities

  • Performs and documents a complete and comprehensive medical nutrition therapy plan in accordance with scope of practice, policies and procedures, national standards of care, as well as consideration of the needs, barriers, and skills/abilities/resources/support system of each patient service in the outpatient setting.
  • Coordinates the nutritional plan of care for outpatients as part of the diabetes education program, and supports the overall established diabetes educational curriculum.
  • Provides counseling in an effective and compassionate way, using active listening techniques and professional behavior, individualizing the care of each patient, setting mutually agreed upon goals, performing follow up activities.
  • Works with RNs and all members of the team in the department to provide individual and group classes for all types of diabetes related services, including Diabetes Prevention Program.
  • Crosstrained to provide diabetes education services as need arises.
  • Accountable for measurements of effective and successful education to patients through measured goals and outcome reports, audits, and satisfaction survey/feedback.
