About The Position

The Outpatient Care Coordinator is responsible for collecting and processing patient information to ensure safe, efficient, compassionate, and competent care delivery in a timely manner. This role requires advanced assistant skills, clinical knowledge, and sensitivity to patients’ needs. The coordinator assesses patient needs for referrals to other care team members such as social workers, clergy, dieticians, physical/occupational therapists, the American Cancer Society, other outside agencies, and Medical Practices. Additionally, the Outpatient Care Coordinator facilitates communication and workflow within and between the infusion center, radiation department, and other departments of HMC, as well as with external referral sources and medical practices. The position involves assisting in patient treatment and care according to HMC's established methods and procedures. This includes knowledge of scheduling, pre-registration, registration, check-out, and pre-authorization procedures, as well as a thorough understanding of procedure coding for order entry and billing. The coordinator collaborates with the Central Business Office to ensure accurate charge submission to insurance providers and works to resolve denied claims, which may involve transcribing clinical notes or Letters of Medical Necessity. The Outpatient Care Coordinator should be able to facilitate physician/patient interactions within and outside the medical center on behalf of the Oncology Service Line and must be able to obtain and interpret information appropriate for the age group of the patient served. This role also includes maintaining department supplies and equipment, submitting work orders for facility maintenance, and ordering supplies.

Requirements

  • Minimum of 2 years working in an outpatient clinical setting or related setting preferred.
  • Familiarity with medical terminology.
  • Familiarity with coding and preauthorization processes.
  • Familiarity with hospital policy and procedures in the outpatient health care work environment.
  • Effective organizational skills.
  • Written and verbal communication skills.
  • Ability to utilize interpreters as needed.
  • Ability to communicate with diverse types of ethnic and socioeconomic groups of people.
  • Behavior that projects responsible and personal interaction.
  • Ability to utilize EHR/EMR software.

Responsibilities
